Location: Sheboygan Wisconsin

Job Summary: Under the supervision of the Family Care Program Manager or the Family Care Program Supervisor, is responsible for clinical assessment, member-centered care plan development, ongoing care coordina! tion, quality assurance and monitoring of services to program members, who may be frail elders and/ or individuals who have intellectual or physical disabilities.

Location: 615 South 8th Street, Suite 130 Sheboygan

Hours: Full-time, generally scheduled 8AM - 4:30PM, Monday through Friday
Education: Bachelor's Degree in Nursing preferred; current State of Wisconsin Registered Nurse license required.

Experience: Minimum of two years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity required. Two years of community nursing preferred.

License/Certification: A Current Wisconsin Registered Nurse License

Travel: Routine home visits required. Use of personal vehicle and valid WI Driver's License necessary.
