Ensures that service needs are appropriately identified.

  • Responsible for day to day supervisor of the Admissions unit and staff. Monitors and evaluates the handling of incoming calls from referral sources and potential patients. Review Intake staff determination of appropriateness of referrals for admission and ensures appropriate triage is occurring for customer service. Reviews and evaluates requests for service and ensures that eligibility requirements are met prior to assignment of cases to home health and hospice admission staff. Assists with referral processing and patient care as needed.
  • Collaborates with and communicates with departments, community resources and clinical management on referral concerns, admissions, non-admission and complaints. Notifies Director of Home Care or V.P. of Clinical Services in Director’s absence of any patients that may not be appropriate for ! services for payer source and determination of non-admit. Ensu! res internal and external customer’s satisfaction.
  • Performs personnel-related functions: assigns, reviews, and monitors the work of subordinate staff, tracks employee attendance and ensures for accurate time reporting; reviews and approves requests for leave; approves pay increases; interviews and hires staff; monitors and documents employee performance on an on-going basis and provides regular performance feedback to staff; approves personnel actions; documents staff deficiencies and takes appropriate action including coaching, verbal counseling, implementation of progressive disciplinary measures, and recommendations on termination of employment and; conducts performance reviews.
  • Ensures for documentation of insurance coverage (Medicare, Medicaid, third party, and private pay). Verifies that the process for prior authorizations is followed. Ensures insurance policies are verified and information is accurate and complete.
  • Reviews and audits med! ical records and recommends/takes action to resolve problems.
  • Ensures for compliance with state and federal regulations. Develops yearly goals and objectives that promote the agency’s goals and strategic plan. Develops and implements action plans with timelines to ensure goals and objectives are met. Completes and submits reports, records and documentation in a timely manner.
  • Collaborates with Billing to meet reimbursement objectives. Ensures that documentation is completed and submitted for reconsideration of payment requests, amendments and appeals related to central intake staff responsibilities.
  • Implements existing practices, standards, policies, and procedures and recommends changes or new policies as appropriate. Audits documentation of Admission staff to ensure that processes and procedures are being followed. Addresses deficiencies in compliance. Assists in the implementation and communication of new processes and procedures.
  • Re! sponsible for the management of resources. Prepares a preliminary budge! t for review for the unit. Monitors and tracks expenditures, overtime, and other costs. Ensures staffing levels and expenditures remain within budget. Notifies Director of Home Health of any budget concerns.
  • Participates in Quality Assurance and Performance Improvement initiatives. Improves outcomes through referral and Intake processes. Ensures a timeliness of admission and other department performance measures are met. Promotes patient satisfaction by improving survey results as they relate to intake and reimbursement processes.
  • Coordinates and/or participates in community and marketing functions to promote Home Health United and its services
  • Reviews and evaluates requests for service. Ensures for adequate staffing levels. Provides on-going direction and guidance to staff. Ensures that documentation is timely, complete, and accurate. Monitors productivity.
  • Responds to staff issues and concerns or complaints from patients, family members! of patients, referral sources, and others. Follows-up on issues and concerns. Assists in the investigation and documentation of incidents and/or policy violations. Works in conjunction with the Director of Home Care to resolve concerns, complaints, and/or violations.
  • Leads, facilitates and participates in staff, general management, clinical management, department, cross-department, cross-Company, and external meetings.
  • Ensures for staff development and training. Provides education on hospice and home care standards of practice, expectations, and care. Ensures staff continues to meet licensure requirements and maintain licensure. Identifies deficiencies and provides one-on-one education and training to staff. Recommends staff for seminars and provides preliminary approval of training requests. Encourages professional development.
  • Ensures for staff safety. Ensures staff is compliant with Company safety rules and OSHA rules and regulations. Monitors! and ensures that work-related accidents and injuries are reported time! ly and investigated thoroughly. Ensures that preventative and corrective safety measures and/or training is implemented at locations to prevent reoccurrence. Ensures standard precautions are utilized. Monitors conformance with corporate compliance and integrity program and takes action when needed.
Minimum Education & Experience:
Associates degree in Nursing with three to five years progressively responsible nursing experience that includes two years home health nursing or any combination of education and experience that provides equivalent knowledge, skills or abilities.

Two years prior supervisory experience.

Prior clinical admissions experience required.

Bachelors degree in Nursing required within two years from date of hire into the position.

Current State of Wisconsin RN licensure required.

Certification in coding and OASIS required within two years from date of hire.

Knowledge, Skills & Abi! lities:
This position requires travel to meeting locations, referral sources, and patients’ homes.

Must maintain a valid Wisconsin driver’s license.

Must maintain an acceptable driver’s record per company policy.

Must maintain proof of current auto insurance.

Must have access to a vehicle in good working condition.

Physical Demands:
This position rates as MEDIUM WORK as defined by the Dictionary of Occupational Titles.
