JOB TITLE: Nurse
PROGRAM: Outpatient Services

SUMMARY
The OS Nurse is responsible for providing professional nursing services to diverse populations served in OS. S/he provides culturally competent, care and services to clients in OS. S/he works closely with a multidisciplinary team to plan, coordinate, and deliver medication services. Primary direct service duties include serving as duty nurse, working in the medication room, case management, and working with prescribers to coordinate medication services. The OS Nurse will have his/her performance reviewed on a biannual basis using a 360-degree evaluation process. This process will look at both quality and quantity of performance. S/he receives supervision from the JMHC Medical Director and is in consultation with the Clinical Team Manager of OS. S/he will work in consultation with case managers and prescribers. S/he will perform other duties as assigned.

QUALIFICATION
ESSENTIAL

  • Current licensure w! ith the state of Wisconsin as a Registered Nurse.
  • 3 y! ears of experience working with individuals who have either mental health and/or substance abuse problems or other behavior health issues.
  • Possesses knowledge related to psychotropic medications.
  • Demonstrated ability to work collaboratively on teams with prescribers, counselors, and crisis service providers.
  • Demonstrated ability to work in collaborative relationships with a variety of disciplines and philosophies.
  • Demonstrated commitment to provide culturally competent, recovery oriented, and trauma informed services to diverse populations.
  • Demonstrated commitment to community-based services for persons with multiple needs and inadequate resources.
  • Demonstrated commitment to consumer empowerment and to improving the meaningful involvement and participation of consumers throughout the programs’ services.
  • Excellent communication skills.
  • Ability to work both independently and as part of a team.
  • ! Ability and willingness to provide coordination of services in addition to providing direct services.
PREFERRED
  • Bilingual in Spanish or SE Asian language
  • BA/BS degree
RESPONSIBILITIES
A. Delivery of Clinical Services
1.Provide nursing coverage for the med room and for duty nurse responsibilities and work with pharmacy to insure proper and appropriate distribution of medications.
2.Ensure that services are culturally competent, promote the principles of recovery, are trauma informed and promote consumer participation.
3.Assist in the development and implementation of comprehensive individualized treatment plans.
4.Conduct timely reviews, updates, and revisions as required by policy and as needed for each consumer, taking into account cultural and personal attributes and characteristics.
5.Maintain contact with consumers to a degree sufficient to monitor and assess progress in compliance with medication servi! ces and basic case management service.
6.Work closely with counselo! rs and prescribers on the team to assure that medication is integrated into client recovery plans.
7.Monitor medication compliance and for effects and side effects, reporting observations back to the prescriber and team.
8.Evaluate consumer medical and mental status, progress and challenges, and share opinions with the team.
9.Contribute to the functioning of the treatment team, supporting and covering for other team members as necessary.
10.Participate in the arranging for the provision of crisis management services as clinically and programmatically appropriate and necessary.
11.Coordinate clinical services with those of other Center areas and outside agencies and assist in problem resolution around clinical issues.
12.Assist, as necessary, with collection of client information for the Patient Assistance Program.
B. Administrative
1.Participate in Program meetings, training programs and other designated Program functions.
2.Participate ! in Center-wide training programs and community education, as assigned.
3.Complete other assignments/duties as assigned by Medical Director or Clinical Team Manager.
