Responsibilities:
Major Responsibilities: Completes initial assessment of patient including psychosocial, environmental and physiological, self-care, educational, continuing care, and discharge planning factors. Plans and completes direct delivery of safe patient care as ! assigned and required for each patient. Initiates and implemen! ts a written nursing care plan individualized to the needs of each patient within the time frame specified in unit-based standards. Documents nursing intervention and patient response on nursing care plan and patient care notes. Participates in developing the discharge plan with the patient/family, physician, social worker, and other staff members as appropriate. Assesses patient/family education needs, plans and provides necessary teaching, evaluates response to teaching, and documents in medical record. Recognizes significant changes in patient’s condition; takes appropriate nursing action when changes occur. Modifies nursing care plan as required by changing patient needs.
Collaborates with other health care team members to coordinate medical and nursing management of patient care. Provides pertinent and concise reports describing patient’s response to medical and nursing plans of care. Administers medication in an accurate, timely manner and demonstrates knowled! ge of medication’s action, side effects, and administration principles. Manages IV therapy, hyperalimentation, venipuncture, and central line care according to established standards. Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.

Qualifications:
Licensure, Registration and/or Certifica! tion Required: Registered Nurse license issued by the state of Wisc! onsin Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA). May obtain within 6 months based on department discretion. Education Required: Associate's degree in Nursing Knowledge, Skills and Abilities Required: Excellent communication (written and verbal) and interpersonal skills. Ability to develop rapport and maintain positive, professional relationships with a variety of patients, staff and physicians. Demonstrated ability to effectively make critical, independent decisions. Good organization, prioritization and problem solving skills. Ability to multi-task with frequent interruptions.

Desired Qualifications

Two years working in an acute care setting as an RN is desired.

Scheduled Hours:
Schedule details: 7:00am - 7:30pm with every third weekend and rotating holidays
