Job Summary:
The practice of cardiopulmonary rehabilitation shall include assessment, treatment, planning, implementation, education, communication and demonstration of leadership to address the needs of patient and their families, as well as to foster a positive working relationship among professionals and ancillary staff. This person may be caring for infants, children, adolescents, adults, and/or geriatric patients and will have the knowledge and skills to care for the physical and developmental needs of these populations.
Essential Functions:
1. Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists as needed. Reassesses patient’s functional needs, and adjusts care plan as indicated to ensure optimal patient outcomes.
2. Demonstrates the ability to care for infants, children, adolescents, adults, and/or ger! iatric patients and has the knowledge and skills to care for t! he physical and developmental needs of these populations.
3. Designs and implements a plan of care for the patient and significant others based on a thorough assessment.
4. Conducts therapy practice within defined standards of care.
5. Directs the practice of support staff in order to achieve positive patient outcomes.
6. Communicates effectively with other professional and support staff in order to achieve positive patient outcomes.
7. Promotes and contributes to a positive, problem-solving and service care environment.
8. Maintains established hospital and departmental policies and procedures, objectives, customer service guidelines, performance improvement program, safety, environmental, and infection control standards.
9. Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains all required prescriptions and signatures.
10. Enhances professional growth and dev! elopment of self and others through participation in educational programs, current literature, in-service meetings, and workshops.
11. Provides direction and leadership to staff, students and new graduates. Acts as a resource to promote cardiac rehabilitation within the community.
12. Must be able to complete the physical, sensory and mental requirements of the position.
13. Responsible for Point of Care glucose testing by Accuchek Meter.
Additional Responsibilities:
- Additional duties as may be assigned by your director/supervisor.
Current licensure in the State of Wisconsin-Required
Bachelor’s degree in Nursing-Required
Prior experience in cardiac rehabilitation, pulmonary rehabilitation, cardiac stress testing, and exercise prescription-Preferred.
Current CPR certification and AED â€"Required
ACLS certification-Preferred will be required with! in 3 months of hire
Basic computer knowledge and typing skills! -Required
Strong written and verbal skills-Required
Working Conditions:
