This position is located in the Primary Care/Ambulat! ory Care clinic at the Janesville Community Based Outpatient Clinic. A collaborative, coordinated, interdisciplinary approach is used in meeting patients' health care needs. Patient care is provided to a primarily male population ranging in age from early 20's to late 90's and average 59 years of age. Patients receive services in an outpatient setting that produce favorable population based outcomes that are valued by VHA. Such outcomes are at a minimum those specified within VISN scorecards. They include timely access to primary care appointments for treatment of common sub acute or chronic problems, access to urgent care, access to brief observational care (< 24 hours), preventive care and care for common chronic conditions using evidence-based practice guidelines. Patient care data can be obtained from all locations via the integrated computer programs.
Work Schedule: Day Shift, Monday through Friday.
Relocation Expenses : Relocation ex! penses are not authorized for this position.
Physical Requi! rements: Walking, bending, stooping, and some lifting in the direct care of patients. There may also be extended periods of sitting.

- Must possess a current, full, active and unrestricted registration as a Registered Nurse and Nurse Practitioner in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia
- A Nurse Practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a masterÂ's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the America! n Nurses Association or another nationally recognized certifying body
- Must be licensed with prescriptive privileges
- Must be proficient in spoken and written English as required by 38 U.S.C.402(d) and 7407(d)
