We currently have an opening for an Integrated Care Coordinator who will:
Coordinate and manage services provided to care management patients to bring about appropriate care and cost-effective outcomes Assess, plan, implement, coordinate, monitor, measure outcomes, evaluate, and document the needs and treatment plans Advocate for health needs on an individual basis

The selected candidate will have:
Completion of an approved MSW program or an approved AND or BSN nursing program Three years of clinical experience in a behavioral health setting; OR Five years of experience in some component of managed care (Preadmission certification, utilization review, quality assurance, or case manag! ement); OR Equivalent combination of clinical and managed care! experience Experience in behavior health or substance abuse reviews preferred Licensed Social Worker (LSW)
