The Nurse Clinician is a full performance level registered nurse position. The Nurse Clinician systematically and cont! inuously collects and assesses data in collaboration with the multidisciplinary teams of the neurology clinic programs to provide therapeutic care for the patient and/or family. He/she performs all required elements of a nursing assessment including but not limited to physical examination, assessment of readiness to learn, psychosocial assessment, functional assessment and utilization of patient/family specific assessment scales as required. The Nurse Clinician is an active member of multidisciplinary teams and collaborates in the development, execution and evaluation of the multidisciplinary plan of care. The Nurse Clinician considers factors related to safety, effectiveness and cost in planning and delivering care. The Nurse Clinician decision¿s and actions on behalf of patients are determined in an ethical manner.
The Nurse Clinician establishes goals and strategies for meeting continuing care needs of the patient, family, and/or other care provider. The Nurse ! Clinician provides care in collaboration with other nursing st! aff members; he/she delegates and supervises care in accordance with nurse practice acts and the professional nursing role.
The Nurse Clinician utilizes research findings in his/her practice. He/she demonstrates knowledge of and participates in Improving Organizational Performance activities. The nurse clinician acquires and maintains knowledge in nursing practice and assumes responsibility for the professional development of self, other nursing staff and students.
Qualifications
Education:
1. Diploma or Associate Degree in Nursing with relevant continuing education required
2. Baccalaureate degree in nursing or higher preferred
Work Experience
1. One (1) year clinical nursing experience with patients with neurological disorders, rehabilitation, ambulatory chronic disease management, or ambulatory specialty care required
2. Three to Five years clinical nursing experience with two of those in the ambulatory clinical ! setting preferred
3. Demonstrated competence with telephone triage and telephone management of patient problems preferred
4. Experience with prior authorization of medical treatments, disability and insurance forms preferred
5. Program coordination and protocol development preferred
6. Experience with electronic medical record preferred
Licenses & Certifications
1. RN License in the State of Wisconsin required
2. Current CPR Certification required
3. Certified Neurology Nurse, Certified Ambulatory Care Nurse or Certified Rehab Nurse preferred
Required Skills, Knowledge, and Abilities
1. Keyboarding @30 wpm
2. Ability to use Microsoft office suite
3. Excellent communication skills
4. Demonstrated ability to multitask, prioritize and delegate
5. Demonstrated ability to be an effective member of a multidisciplinary team
6. Self directed, self starter
7. Demonstrated creative problem solving skills
8.! Telephone assessment and triage
9. Demonstrated ability to develop! content and deliver patient/family education
10. Demonstrated ability to develop patient care protocols based on recognized standards of care and research findings
11. Knowledge of chronic disease and its effects on the patient and family
Physical Requirements
1. Medium: Ability to lift up to 50 pounds maximum with frequent lifting/and or carrying objects weighing up to 25 pounds
Pay
This is a Represented position and falls into Pay Grade 6FR. The salary range begins at $29.08 per hour. Education, experience, tenure and internal equity may be taken into consideration when job offers are extended.
Work Schedule
Monday through Friday between 8:00am and 6:00pm, 24 hours per week.
