The Program Nurse Manager is accountable for program oversight and development for all keys areas of responsibility. S/he is skilled in case management of complex, medically acute patients. S/he utilizes problem solving techniques, process improvement and team development to ensure quality services, patient care and a seamless patient experience. The position establishes standards of communication for all program staff with physicians, the primary care medical home team, hospitals, alternative living programs and community resources.The Program Nurse Manager is responsible for the education, staff development, quality improvement and outreach activities of this program. S/he utilizes evidence based knowledge to plan clinical care and work processes for the Complex Case Program. The Program Nurse Manger develops and facilitates a collaborative working environment for multiple disciplines working in the program, including RNs, Social Workers, Nurse Practitioners and suppor! t staff. S/he collaborates with the Patient Resources Manager ! regularly. In addition, s/he creates, evaluates, and modifies new work processes/standards in this new program and is adept at developing detailed, consistent processes across the continuum of care. S/he creatively develops and monitors operational processes to assess throughput, clinical outcomes, cost effectiveness and patient/family satisfaction. S/he actively markets this program throughout all of UW Health and the associated care programs and facilities where medically homed patients throughout UW Health receive care. S/he is also accountable to ensure processes and systems are in place to attract, recruit, train and sustain a competent clinical staff workforce. S/he is held accountable for meeting department and organizational goals that strive for optimal work processes and the highest quality of patient care.
Qualifications
Bachelors in Nursing RequiredMasters degree in Nursing or Health Care related field requiredMaintain state RN licensure in state wh! ere employedMaintain CPR certification1 year management in a clinical setting required, 3-5 years preferredProgram development experience requiredAbility to develop, plan and monitor program based on data3-5 years of clinical care of medically complex patients requiredCase management experience highly desirableCertification in Case management highly desirableExcellent customer service and assertiveness skillsClinical experience in ambulatory care strongly preferredExcellent oral and written communication skillsAbility to work with all levels of faculty, staff and management.Ability to work independently and prioritize workload to complete projects with minimal directionProven leadership and management abilitiesAbility to work with a diverse populationAbility to work in a fast paced environment and handle stressful situationsAbility to create and present reports related to details of the program including case load, interventions, outcomes, etc.
Schedule
This is a fu! ll-time, salaried position. Hours are Monday - Friday 8:00 a.m. - 5:00 ! p.m.
