Description: Wheaton Franciscan Healthcare is at present looking to employ Registered Nurse, RN - Cardiac right now, this position will be placed in Wisconsin. Further informations about this position opportunity please read the description below. The RN - Patient Care Coordinator position is a registered nurse. The Patient Care Coordinator is responsible to provide care in accordance with the America! n Nurses Association Standards of Practice and other standards established by profession/professional groups, regulating bodies/agents. Nursing care will be delivered in accordance the ASHS Standards of Care/Practice and the Tenets of Care.

The Patient Care Coordinator is responsible for the provision and coordination of nursing care to include the collection and analysis of assessment data in determining nursing diagnosis, identification of expected outcomes and development of plan of care in collaboration with the patient/family and other interdisciplinary team members as appropriate. The Patient Care Coordinator identifies interventions to attain expected outcomes, implements the interventions identified in the plan of care, evaluates the progress of the patient/family toward attainment of outcomes, and documents the process. This care includes sensitivity to age specific and unique needs of the patient/family.

The Patient Care Coordinator promotes and ! evaluates the effective functioning of the health care team. T! he Patient Care Coordinator collaborates with health care team members to meet the identified needs of the patient/family throughout the episode of care. The Patient Care Coordinator delegates tasks and activities as appropriate.

This is a full-time, 3rd shift position, working 36 hours/week from 7:00pm - 7:30am. Schedule varies, weekend rotation required.
