RN Care Manager

Location: Milwaukee Wisconsin

Description: Community Care is currently interviewing RN Care Manager right now, this vacancy will be placed in Wisconsin. Job Summary: Under the supervision of the Family Care Administrator or Family Care Program Manager is responsible for clinical assessment, member-centered care plan development, ongoing care coordination, quality assurance and monitoring of services to program members, who may be frail elders and/or adults with developmental and/or physical disabilities.

Location: 1555 S. Layton Blvd., Milwaukee

Hours: Full-time, Monday â€" Friday 8:00 to 4:30 Education: Bachelor's Degree in Nursing preferred; current State of Wisconsin Registered Nurse license required.

Experience: Minimum of two years recent experience with older adults or persons with physical disabilities in a nursing capacity required. Two years of community nursing preferred.

License/Certification: A Current Wisconsin Registered Nurse License is required.

Travel: Routine travel to Community Care sites and members homes required. Requires use of personal vehicle.
