Heartland Hospice a subsidiary of HCR ManorCare who provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy, is currently searching for a PRN Nurse Practitioner. This position will involve traveling in the Stevens Point and surrounding areas. The Per Diem Nurse Practitioner will provide medical assessments regarding hospice eligibility and report findings back to a physician. The NP will work as a member of the interdisciplinary team as applicable to effectively designate processes for meeting the medical, psychosocial and spiritual care needs of the patient. Educational Requirements: Currently licensed as RN in state; Masters Degree in Nursing and certification through a national Nurse Practitioner licensing board. Position Requirements:
