Care Coordinator / Case Management-Utilization

Location: Waukesha Wisconsin

Description: ProHealth Care is at present looking to employ Care Coordinator / Case Management-Utilization right now, this vacancy will be placed in Wisconsin. For detail informations about this vacancy opportunity please read the description below. The Care Coordinator works collaboratively with patients, nurses, social workers, physicians, other practitioners, caregivers and community agencies in assessing, planning, facilitating and evaluating timely coordination of quality, cost-effective care across the continuum. She/he works flexible hours and has 24-hour accountability as necessary to meet patient/family care needs.

Requirements:
Registered Nurse (RN) or persons with commensurate experience is required. Minimum Experience of 3-5 years in a health care setting. Hospital experience with previous care coordination or case management experience preferred. Minimum Education of RN, Licensed Social Worker, or persons with commesurate background. Bachelor's Degree/Masters - Nursing, Social worker preferred.

Location: Waukesha Memorial Hospital, with some occasional hours at Oconomowoc Memorial Hospital
