Job Summary: Under the supervision of the assigned Regional Director or Family Care Administrator, manages the daily operations and case management activities of assigned Family Care and Family Care Partnership Programs. Responsible for direct supervision of Care Managers and RN Care Managers.
Location: Outagamie
Hours: Full-time, Monday through Friday, 8:00AM to 4:30PM. Education: Bachelor's Degree in human services field including social work, psychology, nursing, or related field required. An Associate’s Degree in a health care or human service field including nursing, social work, psychology, plus a minimum of two years of management experience in addition to the experience specified may be substituted for the bachelor’s degree.
Experience: Four years experience in human services delivery with two years case management of older adults or persons with physical or developmental disabilities and one year supervisory experience required.
License/Certification: No license required.
Travel: Rountine travel to other Community Care sites and participant homes required.
