The Clinical Pharmacist - Acute Care is responsible for providing medication therapy management services in collabora! tion with other providers to assure that drug therapy is safe, effective, efficient and cost-effective. This is accomplished by participating in the following activities: interdisciplinary patient care rounds, medication reconciliation, medication therapy monitoring, review of medication orders, patient education, providing drug information to other health care professionals, scholarly activity, coordinating drug distribution, and overall quality assurance of pharmacy services, The Clinical Pharmacist - Acute Care will provide training and education to Pharmacy Residents and PharmD students through practical and didactic experiences. Incumbent acts independently within the scope of their license and areas of expertise with the help of general policies, procedures and practice guidelines with support from the Pharmacy Manager. This position has independent responsibility for organizing and planning their work in order to accomplish their daily responsibilities as well as bro! ad goals and objectives.

Qualifications

1. P! harmD degree preferred
2. Completion of an ASHP Accredited Residency training program preferred
3. Three years of institutional experience (hospital¿acute care, nursing home, home health care, integrated health system) required
4. Licensed or eligible for licensure to practice pharmacy in the State of Wisconsin required
5. Pharmaceutical knowledge and skills to provide care to adult and adolescent age groups
6. English communication skills, both verbal and written required

Pay

This is a Non-Represented position and falls into Pay Grade 8C. The salary range begins at $88,010 per year. Education, experience, tenure and internal equity may be taken into consideration when job offers are extended.

Work Schedule

Shifts are Monday through Friday between 6:30am and 11:00pm with weekends and holidays required. Hours may vary based on the operational needs of the department.
