TITLE: PT
SUMMARY OF POSITION: To provide physical therapy services in accordance with policies and procedures and in compliance with state and federal regulations. Direction and supervision of physical therapy assistants as indicated.
Essential Functions (But not limited to)
1. Perform evaluation and treatment of movement disorders in accordance
with physician orders.
2. Treats all residents, visitors, and staff with courtesy.
3. Compliance with federal and state regulations, professional guidelines and
code of ethics for the professional of physical therapy and provision of
physical therapy services.
Essential Qualificati! ons (But not limited to)
Graduate of an accredited entry level! physical therapy program.
State credentialing/licensure as required.
Synthesis of knowledge related to anatomy, physiology, and pathology for effective evaluation and treatment of movement disorders.
Sound clinical judgment relating to the provision of physical therapy services.
NOTE: This is not a Rehab Aide or CNA position. At a minimum, You must have graduated from a 4 year bachelors degree program accredited in the area of Physical Therapy AND hold a state license as a PT. New grads from these programs welcome to apply while you wait for your temp license.

POSITION SUMMARY:
To pr! ovide physical therapy services in accordance with EHSI/ProStep policie! s and procedures and in compliance with state and federal regulations. Direction and supervision of Physical Therapist Assistants as indicated.
ESSENTIAL FUNCTIONS:
1. EMPLOYEES: Supervision/Orientation/In-servicing
Participates in the orientation and training of new staff
Participates in the supervision of assistants/students/volunteers and support staff as appropriate
Assists in new employee orientation as requested by management
Supports and participates in the On-Boarding process
Presents clinical in-services to colleagues and/or facility staff as requested by management

Develops and implements treatment plan congruent with evaluation findings, customer goals, clinical judgment and physician orders

Maintains complete, accurate, and up-to-date time records via the SMART system
Writes daily and weekly progress notes, supervisory notes and monthly summaries in accordance with Pro-Step policies and protocols

Participates regularly in customer re! lated team meetings and attends meetings and in-services as directed by the FRC
Prepares customer’s discharge plans in coordination with other team members

MINIMUM QUALIFICATIONS:

Knowledge of federal and state regulations and professional guidelines as relates to delivery of physical therapy services. Ability to read, write, speak and understand English. Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers and other facility staff. Possess special interest in, and a positive attitude about, working with long-term care residents and the elderly. Meets all health requirements, as required by law.
ENVIRONMENTAL AND PHYSICAL REQUIREMENTS:
The responsibilities of this position involve significant physical activities including standing, lifting (up to 60 pounds unassisted), bending, stooping, pushing, pulling and twisting.
