Position Summary:
The Anticoagulation (AC) Clinic Nurse monitors patients on anticoagulation therapy referred by their attending physician. The Anticoagulation RN is responsible for educating the patient throughout the course of anticoagulation therapy, reviewing treatment plans, adjusting warfarin dosing according to AC guidelines and ensuring patients adherence. The goal is to minimize risks associated with AC therapy through education, monitoring, and communication.

Qualifications:
Required:
Registered Nurse

Wisconsin State Licensure.
3 years of nursing experience.

Excellent communication and customer service skills.

Strong critical thinking and problem solving skills.

Preferred:
BSN

Medical/surgical, Cardiace or Orthopedics experience.

Responsibilities:
Educate and monitor patients on anticoagulation th! erapy.

Adjust doses of anticoagulation medication to maintain therapeutic goals.

Order or perform laboratory tests needed to manage the patient’s drug therapy.

Maintain documentation and communication to keep the referring physician informed of their patient’s status.

Consult with the referring or supervising physician regarding problems or complications that require medical decisions.

Provide service that maintains high customer service levels.

Remain current with AC literature and suggest to the AC Pharmacist changes to improve service.

Provide patient education and communication at a level the patients understand so that they may assume an active role in preventing AC related problems.

Provide AC and other drug information as requested by patients, providers and other healthcare professionals.

Maintain accurate clinical and billing documentation.

Maintain data to monitor outcome! s.

Salary Range
The minimum hourly wage is $23.24. Several factors are considered when determining final compensation.
