MAJOR RESPONSIBILITIES (This may not include all duties assigned.) Facilitate communication and interactions between Referring Facility and HSHS/Prevea Facilities. Connecting Referring provider with appropriate HSHS/Prevea staff to coordinate to best meet health care needs. Coordinate with HSHS facility/providers to ensure that patient is ready to be received at their facility Developing and updating Access One protocols to coordinate the process between HSHS/Prevea a! nd all referring facilities. Follow-up with Referring facility! /provider after transfer to ensure referring facility needs were met and update on patient status.
Telephone triage. Give patients telephone advice using nursing judgment within the scope of license. Communicate with providers about patient calls received. Triage patients to the appropriate setting if there is a need to be seen.
Provide patient education. Report test results to patients and instructs as to the appropriate follow-up. Call in prescription refills as directed. Assist in scheduling patients.
Assist with scheduling Access One patients with requested specialty in a timely manner. Communicate with hospitals and other offices as directed by providers. Perform related work as required/ requested by the supervisor/manager. NON-ESSENTIAL DUTIESOther duties as assigned within scope of practice.TYPICAL PHYSICAL DEMANDSWork requires physical hand dexterity and full range of motion for operation of computer.
Work requires excellent com! munication skills. Requires good critical thinking skills. Requires sitting for long periods. Requires ability to hear and see within the normal range.
Requires ability to work under stressful or emergency situations with high concentration and energy level. TYPICAL WORKING CONDITIONSWork is performed in an office environment. Involves phone contact with a various array of patients, employees, and providers. Occasional pressure due to multiple calls and emergencies.
Required Skills EDUCATION / EXPERIENCE RN with current Wisconsin license. Current CPR Certification. NON-ESSENTIAL DUTIESOther duties as assigned. Required Experience Good communicatino skills and previous acute care setting experience a plus.
