Required Education: Graduate Degree Required Experience: 1 to 20+ years Required Security Clearance: None Related Categories: Healthcare â€" Nurses/RNs/Practitioners Position Description Lakeshore Medical Clinic is looking for a APP Advance Practice Provider with at least two years experience in a clinical or hospital environment as a licensed or registered nurse or APP. Coumadin management experience a plus, family practice experience preferred. Qualified candidates will have a master's degree in nursing and a graduate of an accredited Master's Degree Nurse Practitioner/Advance Nurse Prescriber Program.
Currently hold or be eligible for licensure in the State of Wisconsin, and hold a DEA license (or eligible) and CPR Certification. This position is full time and will be located at our Layton (St. Francis) clin! ic. 20 of the 40 hours will be Mondayâ€"Friday, with Primary Care overflow.
The other 20 hours will be evening hours at Urgent Care and every other weekend once Urgent Care opens.RESPONSIBILITIES: * Provide primary care, physical examination and preventative health care services. * Initiates treatment for patients with commonly occurring health problems, which lie within the APP/nurse practitioner's scope of practice. * Prescribes medications. * Initiate laboratory, radiologic and other diagnostic modalities.
- Contacts patients with test results. * Initiate a comprehensive plan of intervention, treatment, consultation and followâ€"up. * Documents findings of patient visits. â€"SOAP note.
- Evaluate patient's physical and emotional reactions to prescribed treatment. * Assist patients in identifying and utilizing resources designed to improve and maintain health. * Promotes patient relations, anticipates and responds to the needs of others and! will provide assistance in a courteous quick manner, treats others wit! h care and respect maintaining privacy, confidentiality and dignity. * Follows all HIPAA guidelines.
- Assist with quality improvement. * Assist physicians in providing quality health care * Exercises effective professional judgment. * Maintains a professional appearance and clean safe environment. * Monitors patients with stabilized long term or chronic illness and aids and supports individuals and their families during the grieving process * * Hospital activities that may include beside rounds with the MD and participating in discharging of the patient * * Nursing Home activities to include making rounds* * Take after hours on call*
