Resource Registered Nurse (9.13)

POSITION SUMMARY: The Resource Registered Nurse (RN) position serves as a clinical resource within the ambulatory clinic through the provision of excellent ambulatory nursing care with a patient centered approach to all interactions. The Resource RN is provides care that is patient and family centered, ethical, evidence-based, safe innovative, and compassionate. The Resource RN provides anticipatory guidance, patient! education, telephone triage, and RN assistance. The incumbent! serves as a backup to provide coverage in the absence of patient care team staff including the RN Team Coordinator and to assist when patient care team needs exceed team staffing resources. The Resource RN assesses patient needs, provides nursing care to meet patient care needs, acts as a resource to team patient care staff, and participates in departmental support functions. The resource RN possesses strong clinical, education, and advocacy skills. The incumbent works under the general supervision of the Clinic Manager. Provider staff may delegate activities to a Registered Nurse within the licensing and RN scope of practice within the state of Wisconsin.

NOTE: This position is regularly exposed to blood borne pathogens, diseases, and cleaning/sterilization solutions.

JOB SPECIFIC MINIMUM QUALIFICATIONS

1.Current licensure as a registered nurse in the State of Wisconsin is required.

2.Bachelors degree in Nursing from an accredited college or university is preferred but not required.

3.A minimum of (1) year of professional nursing experience is highly preferred. Experience working in an ambulatory or ! clinic setting is preferred and will be evaluated if not in a professio! nal nursing role.

4.Possession of or eligibility for a valid Healthcare Provider CPR/AED certificate is required. Employee must obtain certification within four (4) months of employment.

5.Ability to travel to various work sites is required. Employee is expected to provide personal transportation. Possession of a valid, Wisconsin driver license if the employee is operating a motor vehicle is required.

6.Knowledge of scientific nursing principles, methods and techniques in an ambulatory setting as a basis for quality decision-making ability is required. Current knowledge of and maintenance of nursing trends, techniques and certifications is required. Participation in pertinent continuing education courses is required.

7.Excellent problem-solving and analytical skills are required.

8.Ability to perform a variety of clinical skills which may include but are not limited to IV Therapy, injections, allergy shots and wound care is requi! red. Ability to assist in procedures performed in clinics is required.

9.Ability to recognize and identify symptoms and problems, with good nursing assessment skills is required.

10.Ability to be resourceful and flexible in emergency situations is required.

11.Ability to review new trends and techniques and update knowledge base is required.

12.Ability to educate patients and/or families as to the nature of disease and to provide instruction on proper care, treatments and follow-up is required.

14.Excellent assessment skills and the ability to prioritize changing patient conditions is required.

15.Knowledge of interviewing practices and techniques used in order to gather information is required.

19.Knowledge of medical records and appropriate documentation is required.

20.Basic understanding or ability to learn about reimbursement/insurance practices for medical care is required.

21.Ability to maintain good working relationships with patients and staff is required.

22.Ability to maintain patient confidentiality is required. Knowledge of HIPAA requirements.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1.Perform nursing care and administrative duties to support quality patient care. 75%

a.Perform nursing care and related duties, as necessary to insure continuity and quality of patient care.

b.As assigned, float to patient care teams to serve as backup and provide coverage during staff absences, position vacancies or when patient needs exceed staffing resources. This may include RN, CMA or LPN coverage

c.In the absence of the RN Team Coordinator, provide daily operational supervision to CMAs! and LPNs on the care team.

d.Float to other clinics as needed and directed.

Responsibilities include but are not limited to:
Independently manage patient care through the use of protocols.

a.Provide nursing care and assist patient care team, as assigned.

b.Work independently under the strict guidance of established protocols to provide safe and appropriate patient care.

c.Perform and accurately document in-depth patient assessments.

d.Recognize and act appropriately when it is necessary to deviate from established protocols. Develop individualized plan of care.

e.Implement plan of care, providing expert nursing care and including selected activities of medical practice delegated through the protocols. Systematically evaluate the care to determine its effectiveness.

f.Responsible for acting as resource to patient care team staff. Provide screening, initial evaluation, and nursing treatment of! patients in health clinic.

g.Help walk-in patients to determi! ne if they need to be seen immediately or can be sent home with home care instructions and future appointment.

h.Perform patient assessment.

i.Perform routine and complex nursing skills, such as, but not limited to, vital signs, nebulizer treatments, dressing wounds, Non-stress testing, IV starts and catheterization.

j.Administer treatments to patients within parameters of RN license.

k.Initiate and monitor intravenous therapy as ordered by the provider.

l.Provide basic home care instructions to patients in clinic, i.e., crutch walking, nebulizer use, self BP monitoring, bandage changes and repeated wound care instructions, etc.

m.Participate in clinic emergencies per the Emergency Care Policy. Perform first aid or CPR if necessary.

n.Serve as allergy nurse for maintenance allergy injections after appropriate training.

o.Act as resource to Patient Care Services team.

Provision of over-the-phone! and in person patient education and functioning as a patient advocate.

a.Determine the need for home care instructions, anticipatory guidance, and appointments - when and with whom, and emergency care and instructions.

b.Document nursing assessments and patient instructions in the electronic medical record (EMR).

c.Provide appropriate self-care education (e.g. Fever management, lactation advice) with patient feedback to verify comprehension.

d.Make follow up calls as deemed necessary to assure home treatment has been helpful and/or health concern has been taken care of.

Provision of triage via phone and in person, and coordinate appointment scheduling as necessary.

a.Triage messages as received, determining the need for home care instructions, appointment and emergency instructions.

b.Schedule appointments as necessary.

c.Confirm open time with patient.

d.Enter appointment into the computer sy! stem and confirm appointment with member.

Responsible for fulf! illing other team patient care needs as necessary to maintain patient centered customer service and smooth patient flow.

a.Escort patients to exam rooms and prepare patients for exam.

b.Take and record vital signs, patient histories, and any pertinent information relating to encounter.

c.Secure consents, witness signature, and review charts competence.

d.Prepare instruments for exams and surgical procedures.

e.Prepare patients and assist the physicians as needed with procedures.

f.Order and return medical charts to Health Information Department as necessary.

g.Complete requisition slip and assist with scheduling of services ordered.

h.Call in medications to the pharmacy per provider and document in the medical record.

i.Answer any incoming calls in nursing stations as back up.

j.Routinely check In-basket folders and follow-up on messages as appropriate.

k.Follow-up on messages! utilizing other providers to sign scripts, referrals, etc.

2.Participation in departmental support functions. 15%

a.Assist the Clinic Manager and RN Team Coordinator on projects, assignments, as assigned.

b.As assigned, responsible for screening In-baskets for abnormal results. Follow-up with provider and/or members according to established protocols and procedures.

c.Manage float resources for assignments to assigned clinic or other clinics as needed.

d.Participate in Quality Improvement Projects, as assigned.

e.Train, mentor, and/or precept new and existing staff as assigned by Clinic Manager or Chief Nursing Officer.

f.Contribute to a positive morale and good working relationship with medical, patient care, clerical and other clinic staff.

g.Participate in group patient education/health promotion programs.

h.Participate in committees and nursing staff education.

i.Rotate among team! s as directed to maintain adequate staffing.

j.Assist with rev! iew and revision of triage and other nursing care protocols.

k.Develop knowledge of GHC-SCW and positive attitude toward the total organization to better assist in utilizing GHC-SCW services.

l.Offer input regarding the performance of Patient Care Services Staff for the performance review process.

m.Participate in planning and evaluating care for groups of patients in quality teams.

n.Implement data based improvements and care.

o.Participate in on-call rotation including evenings and weekends, as directed.

p.Assist with training of new staff as directed by Clinic Manager or Chief Nursing Officer.

q.Assist in process improvement projects as directed by Clinic Manager or Chief Nursing Officer.

r.Continually look for ways to improve the efficiency and effectiveness of the work processes in assigned area. Communicate ideas to Clinic Manager or Chief Nursing Officer.

3.Performance of other duties as! assigned by a Clinic Manager, Care Team Provider or Chief Nursing Officer. 10%
