Froedtert Health is currently looking for a Staff Pharmacist for our Community Memorial Hospital in Menomonee Fal! ls. In this position you will be responsible for all aspects of pharmacotherapy on an assigned patient care unit and provide clinical support and distribution services in coordination with physician and nursing services in delivering quality and cost-effective drug therapy to patients. Other duties as assigned.

Qualifications:

• B.S. or Pharm. D. degree received from and ACPE certified school of pharmacy.

Successful completion of ASHP Pharmacy Practice or specialty residency preferred.

• At least one year of experience in a hospital pharmacy practice is required. One year residency program in ASHP accredited pharmacy residency program and/or formal training in clinical and drug therapy monitoring is preferred.
• Bachelor's degree in Pharmacy is required.
• Active Wisconsin pharmacist license (or eligible for); 30 CEU per two years as outlined in the Continued Competency policy. American Heart Association (A! HA) Basic Life Support for the Healthcare Provider (BLS) or an! AHA approved equivalent is required within 90 days.
