Nurse Practitioner/Physician Assistant

Provides primary or specialty health care se! rvices within defined scope of practice in collaboration with a licensed physician and in accordance with MCW/FMLH/CHW policies and procedures.

Department Position Purpose

The Advanced Practice Provider (APP) works in collaboration with a multidisciplinary health care team providing Family Medicine care in the outpatient setting.

Requires a strong foundation in evidenced based practice and ensures an environment of safe quality care. The APP demonstrates an advanced level of medical and nursing knowledge, clinical/technical competence, sound clinical judgment, professional communication skills, timely and compliant documentation, and is responsible for professional development and competency validation.

Core Responsibilities

Department Specific Responsibilities

1. Performs comprehensive histories and physical exams, identifies normal and abnormal findings, including physical, cognitive and psychosocial status.

2! . Orders and interprets laboratory and diagnostic tests.
3. Develops pharmaceutical and non-pharmaceutical treatment plans.

4. Monitors effectiveness of plan of care.

5. Educates patients on disease and treatment plans.

6. Performs procedures as trained and appropriately credentialed.

7. Documents within the Electronic Medical Record (EMR) and completes other necessary documents.

8. Collaborates and/or consults with physicians and other health care providers as appropriate.

9. Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors.

1. Provides care for Walk-In Clinic patients.

2. Performs and documents history and physicals on adults; orders and interprets diagnostic tests, establishes a presumptive diagnosis, implements a therapeutic plan including appropriate follow-up plan, writing prescriptions

3. Collaborates with interdisciplinary health care team.

4. Documents all care in EpicCare, the electronic medical record system.

5. Provides preventive health care services to patients. Guides patients in such areas as routine vaccinations and common and non-routine illnesses (acute and chronic). Provide counseling and instruction to patients regarding the patient’s health related problems.

6. Accurately, and promptly documents services provided to patient in the EMR and bill for services based on services provided and medical need for services.

7. Facilitates and communicates with patient, family and staff to promote continuity of care across the inpatient continuum of care. Participates in family meetings.

8. Follow-up with patients regarding test results. Assists with patient education materials and participates in developing patient teaching plans. Assists with development of clinic protocols

9. Triages patient telephone calls and provides consultation.

10. Maintai! n clinical competence through continuing education, attending conferenc! es, etc. Acquire information on the most current developments and trends in the primary care field, through educational programs, in-services, professional associations, and literature

Qualifications

MINIMUM POSITION QUALIFICATIONS

Education Requirements

Advanced Practice Nurse

Master’s Degree

Experience

  • Zero years of experienced required
  • Outpatient Primary Care/Walk-In Clinic experience preferred.
Certifications/Licenses

  • Wisconsin RN license
  • Wisconsin Advance Practice Nurse Prescriber License
  • Certification as Advance Practice Nurse
  • DEA Certification
  • Certification as a Family Nurse Practitioner

Bachelor’s Degree or higher in Physician Assistant Stud! ies

Experience

  • Zero years of experienced required
  • Outpatient Primary Care/Walk-In Clinic experience preferred
