Advanced Practice Nurse

Provides primary or specialty ! health care services within defined scope of practice in collaboration with a licensed physician and in accordance with MCW/FMLH/CHW policies and procedures.

Department Position Purpose

Works closely with staff physicians, fellows and residents to provide excellent care to the Transplant surgery patients.

Physician Assistant

Provides primary or specialty medical care within defined scope of practice under the supervision of a licensed physician and in accordance with MCW/FMLH/CHW policies and procedures.

CORE RESPONSIBILITIES

1. Performs comprehensive histories and physical exams, identifies normal and abnormal findings, including physical, cognitive and psychosocial status.

2. Orders and interprets laboratory and diagnostic tests.

3. Develops pharmaceutical and non-pharmaceutical treatment plans.

4. Monitors effectiveness of plan of care.

5. Educates pat! ients on disease and treatment plans.

6. Performs pro! cedures as trained and appropriately credentialed.

7. Documents within the Electronic Medical Record (EMR) and completes other necessary documents.

8. Collaborates and/or consults with physicians and other health care providers as appropriate.

9. Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors.

Department Specific Responsibilities

1. Provides patient care in both the inpatient and outpatient setting for the transplant patient.

2. Coordination of daily rounds with fellow, staff and co-management team for the transplant patient.

3. Participates in preoperative and postoperative care of hospitalized patients.

assisting in the operating room.

5. Educates patients and families on preoperat! ive and postoperative care.

6. Communicates with primary care providers, specialty care providers and/or inpatient providers to provide comprehensive patient care.

7. Coordinates discharges from the hospital to facilitate flow and patient understanding.

8. Develops programs for specialized Transplant surgery treatment algorithms

9. Works to develop surveillance programs for outcome assessment

Qualifications

MINIMUM POSITION QUALIFICATIONS

Education Requirements

Advanced Practice Nurse

Master’s Degree

Physician Assistant

Bachelor’s Degree or higher in Physician Assistant Studies

Experience

  • No experience required- will train perfect candidate
Certifications/Licenses

(Must be current and maintained)

Advanced Practice Nurse

  • Wisconsin RN license
  • Wisconsin Advance Practice Nurse Prescriber License
  • Certific! ation as Advance Practice Nurse
  • DEA licensure
Physician Assistant

  • Wisconsin PA license
