Location: Ashland, Wisconsin

Please note: These positions will be required to telework. The employees must be able to establish a secure home office or alternate work site away from the DQA Northern Regional Office (NRO) location. These positions also require a minimum of 75% travel. These jobs offer a flexible schedule with the option of four 10 hour work days.

Position Summary:

These positions function as members of the survey team conducting program monitoring activities and ensuring compliance with federal regulations and state codes, standards, and guidelines for licensure, program certification, and eligibility for participation in Medicare and Medicaid within nursing facilities and skilled nursing facilities (long term care facilit ies). These positions ensure compliance with standards and rules in the following areas: admissions; transfers and discharges; resident rights; staffing and census; quality of care assessment; quality of life assessment; resident care plans; social, psychological and rehabilitative services; medical services; and therapeutic activity programming. These positions are also responsible for providing expertise to the survey team in the areas identified above and serve as team coordinators as designated. These positions also investigate complaints and conduct follow-up and surveillance visits.

Due to the nature of the position, all applicants who may be appointed to these positions will be required to allow DHS to conduct a background check to determine whether the circumstances of any conviction may be related to the job being filled.

These positions require the ability to drive a State vehicle or the ability to provide one's own transportation. In order to become authorized to drive a State vehicle, you must meet the requirements of the State of Wisconsin Fleet Policy which are:

  • Must have a valid driver's license;

  • Must have minimum of two year's licensed driving experience;

  • Must be 18 years of age.

Additionally, your driving record MUST NOT reflect any of the following conditions:

  • Three or more moving violations and/or at-fault accidents in the past two years;

  • Operating While Intoxicated (OWI)/Driving Under the Influence (DUI) citation within the past 12 months;

  • Suspension or revocation of driver's license.

Qualifications:

A minimally qualified applicant will have:

  • Registration or be eligible* to register as a nurse in the State of Wisconsin by the time of appointment.

  • An ability to travel to requi red sites.

  • Experience working in one of the following health care settings: nursing home care, hospice care, elderly home care, wound care, mental health, intellectual disabilities, physical disabilities, critical care, risk management, quality assurance, or public health.

  • Experience conducting resident assessments, care planning, and/or investigations.

  • Strong written and oral communication skills.

  • Experience using Microsoft Office Suite for data entry and document creation.

  • Eligible is defined as your license application is currently in progress with the Wisconsin Department of Safety and Professional Services or you currently hold a license in another state and are eligible to obtain a license in the State of Wisconsin.

A well-qualified applicant will have experience:

  • With investigative, legal, or regulatory enforcement related to healthcare.
  • Participating in or conducting an inspection or survey.

  • Working with nursing home residents and/or the elderly population.

  • Working in more than one of the following health care settings: nursing home care, hospice care, elderly home care, wound care, mental health, intellectual disabilities, physical disabilities, advocacy, critical care, risk management, quality assurance, or public health.

  • Experience working effectively as a member of a team.
