Location: Wausau Wisconsin

As a Home Health Nurse, in Stevens Point/Wausau WI you will empower patients and families to make their own decisions and focus on increasing patient quality of life. This is accomplished through providing multi disciplinary care to patients in their home. Patient population includes newborn through geriatric.

The incumbent in this position:!
Implements and collaborates with the plan of care.
Submits timely, accurate and consistent assessments and documentation.

Monitors and evaluates patient care outcomes.

Participates in Quality Improvement measures to improve outcomes.

Casual position. Includes one to two holidays per year. One weekend a quarter.

Qualifications

Graduate of accredited school of nursing

Minimum 2 years acute care/medical surgical nursing experience preferred

Minimum of 1 year home health experience preferred

Current Wisconsin nursing license or eligible to obtain

Current drivers license, proof of auto insurance, and vehicle availability

CPR certification or the ability to gain certification
