Description: Group Health Cooperative of South Central Wisconsin is hiring Care Team Specialist-LPN or CMA right now, this occupation will be placed in Wisconsin. POSITION SUMMARY The Care Team Specialist serves as a member of a care team and assists in providing high quality patient care including assisting patients in maintaining their physical and mental well-being. The Care Team Specialist performs technical, clinical, and administrative tasks which contribute to the smooth workfl! ow and quality patient care experience in the clinic. Responsi! bilities include, but are not limited to pre-visit preparation, monitoring patient flow, rooming patients, obtaining patient histories and vital signs, managing certain test results following protocols, supporting Epic in-basket workflow, and returning phone calls. The Care Team Specialist assists and collaborates with the Registered Nurses in the management and coordination of patient care including the review of team/provider in-baskets and My Chart messages for the assigned care team. Practitioner and Registered Nurse staff may delegate assignments to the Care Team Specialist.

JOB SPECIFIC MINIMUM QUALIFICATIONS
1. Successful completion of a Medical Assistant or Licensed Practical Nurse training program from an accredited school or program is required. LPNs must have current licensure or obtain licensure as a practical nurse in the State of Wisconsin within six (6) months of employment. Medical Assistants must be eligible for and receive national certification by the American Association of Medical Assistants (AAMA) within six (6) months of employment and must maintain and complete CME requirements.
2. Possession of or eligibility for a valid H! ealthcare Provider CPR/AED certificate is required. Employee must obtai! n certification within four (4) months of employment.
3. Ability to travel to various work sites. Employee is expected to provide personal transportation. Possession of a valid, Wisconsin driver license if the employee is operating a motor vehicle.
4. One year of relevant experience working in a clinic or outpatient setting is desirable.
5. Knowledge of and ability to perform or learn standard clinical procedures according to established protocols and policies are required.
6. Knowledge of and/or ability to learn how to properly use and maintain equipment, materials and supplies used during the course of treatment is required.
7. Knowledge of and/or ability to learn and use computer based applications such as MS Office, electronic medical record systems and scheduling applications is required. Experience with EPIC is desirable. Must type a minimum of 30 WPM with minimal errors.
8. Knowledge of or ability to learn outpatient clinical administrative pr! ocedures are required.
9. Knowledge of medical terminology and knowledge of basic pharmacology is required.
10. Ability to maintain quality, safety and/or infection control standards is required.
11. Ability to recognize, identify and report problems to appropriate personnel is required.
12. Basic understanding or ability to learn about reimbursement/insurance practices for medical care is required.
13. Knowledge of inventory management practices is required.
14. Ability to educate patients and/or families as to the basic nature of disease and to provide instruction on proper care, treatments and follow-up is required.
15. Knowledge of medical records and appropriate documentation is required.
16. Knowledge of interviewing practices and techniques used in order to gather information is required.
17. Excellent written and verbal communication skills are required.
18. Strong interpersonal skills and the ability to be culturally sensitiv! e and to work with a diverse patient population is required.
19. Ab! ility to maintain good working relationships with patients and staff is required.
20. Ability to maintain patient confidentiality is required. Knowledge of or ability to learn HIPAA requirements.
21. Ability to climb, stoop, kneel and crouch is required. Ability to bend, reach, lift, pull, push and/or carry up to 10 pounds regularly and up to 50 pounds occasionally is required. Ability to stand and sit for prolonged periods of time is required.

Additional Accountabilities: Clinic Manager, Assistant Clinic Manager, Clinical Practitioners (Physicians, Chiropractors, Nurse Practitioners and Physician Assistants) and Registered Nurses.

SUPERVISION: Position/Incumbent does not supervise subordinate staff.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Provision of high quality care to patients. 40%

a. Performs and implements procedures and treatments as delegated by RN or ordered by Practitioner and according to policy and procedure.
b. Assists with surgical procedures using sterile techniques.
c. Administers medications according to policy and procedure. Demonstrates and understands administration techniques, recognizes and reports effects and side effects of medicat! ion and instructs patient appropriately.
d. Provides patient educat! ion for procedures, tests, treatment and care within established parameters. Provides pre-operative teaching to members as assigned. Reinforces the teaching protocols established by the RN or medical practitioner and provides basic health care instruction.
e. Observes and communicates pertinent data to RN/Practitioner.
f. Reviews any recent concerns identified via My Chart or phone.
g. Completes Epic chart review prior to team huddle.
h. Anticipates patient and RN/Practitioner needs based on reason for visit and patient history. Prepares patient properly for visit. Ensures proper equipment and supply set up for visit.
i. Provides assistance in emergencies under direction of RN/Practitioner.
j. Assists RN/Practitioner with patient call backs as directed.
k. Understands advanced and same day scheduling process.
l. Assists with direct patient care examination, procedures and related tasks; escorts patients to examination rooms, assists in obtain! ing patient histories, takes vital signs such as temperature, blood pressure, pulse and general condition, records information and assists with medical exams. Hands instruments, equipment and supplies to examining practitioner.
m. Performs and administers RN/Practitioner ordered routine screening procedures such as electrocardiograms, spirometry, vision, and hearing tests. Performs and administers RN/Practitioner ordered throat cultures and ear lavage, and obstetrics related non-stress tests. Participates in pre-op preventive health exams (PHE’s) by reading and collecting data, history and questions. All procedures are performed utilizing GHC-SCW nursing protocols and procedures.
n. Under established protocols and utilizing sterile techniques, performs routine specialized procedures such as venipuncture and phlebotomy, extracting sutures and staples, cleaning wounds, applying medications and dressings, performing sterile dressing changes, and assisting in minor su! rgical procedures and biopsies.
o. Observes and reports to practiti! oner patient physical and mental conditions.
p. Measures, administers and records selected injections such as immunizations, TB skin tests and antibiotics and watches for reactions.
q. Collects and prepares specimens for laboratory analysis. Screens laboratory test results for abnormal and normal values.
r. Prepares, cleans, and disinfects examination rooms including changing linen and setting up equipment, instruments and supplies for examinations and minor procedures.
s. Maintains stock in exam room of equipment, medicines, and medical supplies as necessary.
t. Performs and implements delegated nursing or medical acts beyond basic nursing care under the direct supervision of the RN or medical practitioner. Accepts only those acts that incumbent is competent to perform.

2. Assists the Registered Nurse with coordination and collaboration of care, responds to in-basket and MyChart inquires and messages. 20%

a. Routinely checks daily in-b! asket folders and MyChart messages and follows up as appropriate per protocol.
b. Returns information/messages to RN/Practitioner, as appropriate, in accordance with procedures and protocols.
c. Gathers data for RN and/or Practitioner which is used in assessment.
d. Follows up on messages and/or reports as assigned or authorized within the scope of practice standards.
e. Reviews provider My Charts and completes, as appropriate.

3. Provision of clinic operational assistance in support of quality patient care. 20%

a. Answers phone calls and routes or handles appropriately. Screens telephone calls related to patient care. Obtains appropriate information with attention to detail and accuracy. Uses set protocols/standing orders as resources to implement appropriate care. Consults with an RN or medical provider regarding situations outside of established protocols.
b. Responds promptly to phone and in-person patient requests according to depa! rtment policy. Directs calls to RN or Practitioner in accordance with e! stablished protocols and procedures. Provides appropriate information in advance of forwarding request to RN or Practitioner, as appropriate.
c. Documents all patient contact in medical records.
d. Opens, sorts and distributes daily mail, as assigned.
e. Coordinates the review and follow-up of phone requests for medical advice and prescription refills.
f. Performs patient follow-up phone calls for the RN or Practitioner, as directed.
g. Schedules patients based on scheduling guidelines. Confirms appointments according to policy and procedure.
h. Performs assigned activities in preparation for patient visits including obtaining needed reports. Reviews medical records for up-to-date immunizations, problem list and other required documentation.
i. Coordinates specialty care for patients including scheduling procedures and assisting patients with scheduling appointments to referred providers. Ensures patient receives proper information for preparatio! n and follow-up.
j. Receives diagnostic reports and refers to physicians for appropriate follow-up.
k. Informs patients of test results per RN/ Practitioner request.
l. Copies and sends pertinent information to specialists, other physicians or other appropriate individuals/groups, as necessary and as requested.
m. Assists RN/Practitioner with patient forms completion.

4. Demonstration of teamwork and personal accountability to Nursing Department. 15%

a. Contributes to the positive morale of department by looking for ways to support others.
b. Participates in daily coordination of care with RN/Practitioners.
c. Participates in huddles.
d. Participates on committees as requested and attends staff meetings.
e. Rotates among teams as directed to maintain adequate staffing.

h. Handles daily workload in pr! ofessional and timely manner.
i. Provides input for evaluations and! peer reviews as requested.
j. Attends required in-services, updates knowledge base, and maintains skills assessment.

l. Maintains accurate time and attendance records.
m. Understands and follows procedures for requesting time off, documenting scheduling changes, and calling in sick.
n. Understands responsibility to work weekend hours and reviews duties for those hours.
o. Routinely checks and reviews GHC-SCW personal email account and respond to email communications, as appropriate.
p. Understands and adheres to Confidentiality Policy and Contract.
q. Adheres to GHC-SCW Nursing Department dress code.
r. Participates in continuing nursing department education courses to update knowledge base and stays current with emerging procedures and technology.
s. Assists in process improvement projects.
t. Continually looks for ways to improve the efficiency and effectiveness of the work processes in assign! ed area. Communicates ideas to supervisor.
5. Responsible for understanding and following established policies and procedures. 5%

a. Knows worksite and on-line locations of all procedure and policy manuals and related documents/materials.
b. Maintains familiarity with all policies and procedures. Reviews policies and procedures on an on-going basis in order to maintain current knowledge and keep abreast of updates and revisions. Seeks clarification of any policies or procedures to insure adequate knowledge and proper practices. This includes but is not limited to: Infection Control and Prevention, OSHA requirements, Employee Safety, Nursing Policies and Procedures and Emergency Management Plan for Insurance and Clinic Operations (EMPICO).
c. Participates in and completes on-line and other training activities such as the annual OSHA review and in-service education.
d. Knows location of Hazard Communication Handbook and Blood-borne Pathogen resources.!
e. Knows where to find and how to use the toll free number for acc! essing MSDS on Demand information.
f. Follows procedure for exposure to blood or body fluids.
g. Follows procedure for handling and sterilization of dirty instruments and equipment.
h. Understands correct storage and maintenance of instruments and equipment.
