Job Responsibilities:
The purpose of this position is to ensure the facility complies with the Federal Medicare Prospective Payment System (PPS) process, and Medicaid reimbursement, as required by professional, federal and state nursing home regulations regarding the Minimum Data Set, Resident Assessment Instrument (RAI), Medicare and Medicaid requirements, and resident care. Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required. Thorough understanding of the Quality Indicator process. Ensuring that documentation in the facility meets Federal, State, and Certification guidelines. The MDS Coordinator will coordinate the RAI process assuring the timeliness and completeness of the MDS and Interdisciplinary Care Plan.
Essential Duties and Responsibilities:
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
⢠Responsible for coordinating and completing required MDS, Care Area Assessments (CAAâs) and plans of care in a timely manner for all residents. These include entry and death tracking MDS, admit, discharge, Significant Change Status Assessment, quarterly, annual, for Medicare â" 5, 14, 30, 60, 90 day, end of therapy, start of therapy, end of therapy resumption, change of therapy.
⢠Keep updated on all new information and requirements regarding the MDS process for Medicare and Medicaid.
⢠Disseminate information and train the interdisciplinary team regarding Medicare and Medicaid MDS process and requirements.
⢠Complete, and monitor completion of required MDS paperwork by the interdisciplinary team.
⢠Schedule MDS requirements so that they are completed in a timely manner by the interdisciplinary team.
⢠Produce accurate resource utilization groups (RUGS) levels to be utilized for billing of services. Provide copies of the RUGS levels to the Business Office, Administrator and Director of Nursing.
⢠Maintain log of Medicare Part A days used for each resident per stay.
⢠Prior to admission and after admission, determine eligibility for coverage of care under Medicare Part A. Communicate with the resident, responsible person, interdisciplinary team and Business Office regarding eligibility.
⢠Participate in discharge planning for residents of the Medicare wing.
⢠Review information and reports utilizing the MDS data, such as the Quality Indicator reports.
⢠Facilitate daily Medicare meetings with the interdisciplinary team, Monday through Friday.
⢠Knowledge of ICD-10 coding is pertinent to sequence diagnosis accurately.
⢠Updating Medicare replacement insurance policies on their clients at least weekly.
⢠Do monthly calendars and MDS schedules for staff. Provide updates throughout month related to admits, discharges, Significant Change Status Assessment.
⢠Doing Medicare certification letters.
⢠Assist in completing and maintaining Medicare physician certification and recertification forms.
⢠Recommend and assist issuing Medicare and Insurance denial letters.
⢠Communicates with the company who provides the MDS/care plan computer software to problem solve and improve the program.
⢠Work with the interdisciplinary team to ensure the residents receive the needed services and cares.
⢠Gathering information for Resident Assessment Coordinator audits.
Physical and Mental Abilities Required to Perform Essential Job:
Language Ability and Interpersonal Communication
Ability to analyze data and information using established criteria, in order to determine consequences and to identify and select alternatives. Ability to compare, count, differentiate, measure, copy, record and transcribe data and information. Ability to classify, tabulate and categorize data.
Proficient in written and oral communication the English language.
Physical Requirements:
Ability to operate equipment and machinery requiring complex and rapids adjustments, such as a computer, photocopier, telephone, fax machine and nursing equipment. Proficient in use of the computer and federal and state mandated MDS software programs.
Ability to exert moderate to heavy, but not constant physical effort, typically involving some combination of walking, sitting, standing, pushing, pulling, carrying, lifting, stooping and balancing.
Ability to coordinate eyes, hands, feet and limbs in performing movements requiring skill and training, such as data entry, examining and transporting residents and assembling.
Ability to exert moderate but not constant physical effort, typically involving some combination of stooping, kneeling, crouching, crawling, lifting, carrying, pushing and pulling.
Mathematical Ability:
Ability to calculate percentages, fractions, decimals, volumes, ratios, present values and spatial relationships. Ability to interpret basic descriptive statistical reports.
Judgment and Situational Reasoning Ability:
Ability to use functional reasoning in performing influence functions such as supervising, managing, leading, teaching, directing and controlling.
Ability to exercise the judgment, decisiveness and creativity required in situations involving the direction, control and planning of an entire program or multiple programs. Demonstrated ability to maintain confidentiality.
Environmental Adaptability:
Ability to work under generally safe and comfortable conditions where exposure to environmental factors such as irate individuals, intimidation and disease may cause discomfort and poses a limited risk of injury.
Job Qualifications:
Degree in Nursing from an accredited college or school of nursing. Required to be a licensed Registered Nurse in Wisconsin. Minimum of one year of work experience as a Registered Nurse. Must have MDS 3.0 and ICD-10 coding experience. Knowledge of the Medicare and Medicaid requirements to complete the MDS program and related resident plan of care. Cardio-pulmonary Resuscitation (CPR) certification required and maintained during employment with Wood County. Certified American Association Nurse Assessment Coordinators required.
