Description: Wisconsin Community Services, Inc. is presently looking of CSP Case Manager/Case Worker III right now, this occupation will be placed in Wisconsin. Provide ongoing assessment and treatment planning, case management, supportive psychotherapy, symptom management, medication monitoring, and crisis intervention including after hours hotline coverage. Work is performed in the office, the community and in clientsâ homes; up to 6 hours a day may be spent driving.
2. Aid clients with activities of daily living to include coaching and hands on assistance in the areas of housing, money management, vocational/educational pursuits, scheduling and tran! sportation to appointments
3. Aid clients with accessing a! ppropriate medical services and following up with medical recommendations to include scheduling and transporting or arrangement of transportation as necessary and appropriate. Follows instructions of the CSP Registered Nurse or proxy with regard to medical follow up of clients.
4. Act as the single point of contact by coordinating the efforts of all professionals and paraprofessionals involved with each client and mobilize resources in and outside the clinic to assist the client in achieving treatment plan goals.
5. Encourage and develop natural supports for each client including family members, neighbors, friends, etc.
6. Maintain accurate and timely documentation in the form of assessments, treatment plans, progress notes, etc. as required by agency policy and HFS 63.
7. Responsible for formulation of appropriate discharge planning with the client.
8. Accurately manage clientsâ âbanksâ and turn in related items [money pouches, receipts, signed ca! sh sheets, etc. in a timely manner.
9. Frequent home visits and transporting of clients in the community using personal vehicle.
1. Volunteer for client related clinic gatherings throughout the year e.g. holidays, summer picnics, etc.
2. Attend and participate in daily staffing and in-services
3. Coverage of assigned secondary caseload for staff absences, and volunteer for client coverage as needed or requested by supervisor
4. Drug testing clients as needed
Job Qualifications:
Knowledge, Skills and Abilities: Good clinical skills especially related to mental illnesses and substance abuse disorders; knowledge of DSM IV, psychotropic medications and their side effects; ability to communicate effectively and in a positive manner with staff and clients; ability to respond appropriately in crisis situations; sensitivity toward cultural, ethnic and disability issues; good organizational and computer and typing skills; effective use ! of time; ability to assist clients with activities of daily living such! as shopping, cleaning and cooking; ability to multitask.
Minimal Qualifications: Masterâs Degree in a Behavioral Science and 1-2 years social work experience; no convictions or license revocations that would fail a caregiver background check; valid driverâs license, automobile and sufficient insurance to meet agency requirements.
