DUTIES:
The Social Worker position is located at the Clement J. Zablocki VA Medical Center in Milwaukee, Wisconsin.
WORK SCHEDULE: Full time, Monday through Friday 7:30 am - 4:00 pm
DUTIES: The Home Based Primary Care (HBPC) Social Worker provides a full range of social work for veterans referred to or admitted into the HBPC program. The Social Worker provides direct services to an adult population in their home environment and independently conducts psychosocial assessments and develops treatment plans. He/she offers psychosocial intervention! s and measures outcomes to interventions and coordinates a wide range of community based services. The Social Worker independently provides case management and crisis intervention services. He/she maintains knowledge of state Medical Assistance long-term care waiver programs and other resources and facilitates referrals based upon need and eligibility. The Social Worker must have working knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illness/injuries, common medications and their side effects, and medical terminology. In addition, the Social Worker independently provides consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to the treatment plan. He/she provides assistance to veterans under the Contract Nursing Home (CNH) program. This includes monthly visits to the vet! eran which involve discussion, observation, and documentation of care p! rovided to veterans during the visits. The Social Worker also serves as a leader of the CNH program inspection team. He/she reviews community nursing homes with VA contracts on an annual basis to recommend continuation of VA contracts. He/she also leads on-site inspections of community nursing homes found to have quality-of-care issues and also participates in performance improvement activities.

Education - A MasterÂ's Degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). A doctoral degree in social work may NOT be substituted for the masterÂ's degree.
Licensure - Individuals assigned as a Social Worker Program Coordinator or Manager must be licensed or certified at the Independent Practice Level (CAPSW).
Grade Requirements:
Minimum of one year of post-MSW degree experience in the field of health care social work
OR
Doctoral degree in social work from an accredited school of social work
AND
Demonstrated Knowledge, Skills and Abilities:
Knowled! ge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services
Ability to independently assess the psychological functioning and needs of patients and their family members and to formulate and implement a treatment plan, identifying the patientÂ's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team;
Ability to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds requiring knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society and culture;
Knowledge and experience in the use of medical and mental health diagnoses, disabilities and tre! atment procedures including acute, chronic and traumatic illness/injuries, common medications and their side effects and medical terminology;
Knowledge of psychosocial treatment and ability to independently implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals requiring independent judgment and skill in utilizing supportive, problem solving or crisis intervention techniques;
Ability to independently provide counseling and/or psychotherapy services to individuals, groups and families within the bounds of your license or certification;
Ability to provide consultation services to other staff about the psychological needs of patients and the impact of psychosocial problems on health care compliance with treatment, and provide orientation and coaching to new social workers and social work graduate students
Ability to ! independently evaluate your own practice through participation peer rev! iew case conferences, research studies or other organized means;
K nowledge and skill in the use of computer software applications for drafting documents, data management, and tracking.

Combining Education and Experience: Combinations of successfully completed post-high school education and experience may be used to meet total qualification requir! ements. It will be computed by first determining the applicant's total qualifying experience as a percentage of the experience required for the grade level; then determining the applicant's education as a percentage of the education required for the grade level; and then adding the two percentages. The total percentages must equal at least 100 percent to qualify an applicant for that grade level.
