As a collaborative member of the clinic medical home team, this role oversees patient workflow, utilizing care algorithms, clinical protocols and advanced intake tools to triage and provide care to patients. Highly committed to team construct.

Commits to the Affinity Medical Home Principles of Practice.

Promotes quality and service excellence for patients by providing personalized care in a collaborative team environment.

Shares responsibility for providing health care for the team's patients in a clinic setting.

Contributes in a positive fashion to all behaviors and activities which support the team model. Accepts ownership and responds effectively to patient, team or personal concerns and works towards positive resolution.

Administers medications and immunizations.

Relays information to providers, and communicates appropriate information back to patient, includ! ing such information as lab results, diagnostic test results, ! and information related to the plan of care as developed by provider.

Anticipates and facilitates collection of information to assure effectiveness of upcoming visits.

Rooms and prepares patients by collecting detailed history and relevant rating scales and gathering information of patients concern. Reviews medications noting any discrepancies.

Completes and documents vitals. Following protocol may complete a portion of the physical exam.

Completes testing according to protocol prior to physician seeing patient. This may include Pulse Oximetry, Monofilament testing, peak flow , etc.

Performs other duties and responsibilties as assigned.

Qualifications

Graduate of an accredited Practical Nursing program and licensed (or awaiting licensure) in the State of Wisconsin, Certification as a Medical Assistant or completion of Medical Assistant program/internship. The MA applicant must be eligible to take the CMA (Americ! an Association of Medical Assistants - AAMA) certification exam. Graduate of a Medical Assistant program that has been accredited by either: Commission on Accreditation of Allied Health Education Programs (CAAHEP) or Accrediting Bureau of Health Education Schools (ABHES). Previous experience working as a LPN, Medical Assistant within a primary care environment preferred. Keyboard skills and experience working with computer based programs required.
